Can gynecomastia return after just 4 weeks post surgery?

Likely, you are feeling residual scar tissue that you are only feeling now after all the swelling has subsided. You are still early in the healing process. It takes at least three months to see the final result. It is very common to feel lumps during the healing process. I instruct my patients to massage but you should check with your own surgeon. Hang in there.

5 weeks post surgery is too early for the gland to re-grow, but there are other potential causes of swelling at this relatively early post-op time-point. You should expect a mild degree of swelling right now, and returning to the gym may make the swelling a little worse initially, but if it is more than that I would suggest you arrange a review with your PS for a clinical examination - it could be a fluid collection that requires drainage. It doesn't sound like anything to worry about though!

Those lumps may be residual breast tissue, new breast tissue, fat or simply scar tissue. In my opinion a scan (ultrasound is the easiest one) will help identify the nature of the tissue and then you can see a surgeon and decide what to do
