Based on your photos, I would highly advise against further surgery. You appear to have very thin nasal skin, and a fourth operation would not guarantee much of anything. Depending on your examination, microdroplets Silikon 1000 injections may be considered for smoothing out your nasal tip appearance so it may look less operated. Hello and thank you for your question. Based on your photographs, I would recommend a revision under general anesthesia. This can address your tip asymmetry, prominent columella and give the tip a smooth and more natural appearance. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
