The first photo is pre-break in 2011 or so, I then broke my nose and had a closed surgery to set it. It then looked like the second photo. I had a septal perforation repair in 2015, though nothing external was done cosmetically. I was annoyed that my nose used to be so smooth pre-fracture, and wanted that again so I went to the Dr. who fixed my nasal septum for a closed “nasal valve repair” procedure in November 2023. My nose now looks like the last several photos. What is wrong with it?

Yes adequate tip support is still possible during your revision case. In my practice I typically use rib cartilage for revision cases. This can be anchored to your remaining septum to provide tip support.
