Can I fix these pinched outer lids?

I’m sorry you are unhappy with your eyelid procedure. Difficult to tell without seeing you in person. Typically, scarring and the “pinched” sensation does improve with time (6 months). Botox would be a good starting point. Depending on your response to the neurotoxin, a temporal browlift could help. You need an in person consultation to discuss your best options.

I looked at your photo before reading your question, and it was not readily obvious to me that you had blepharoplasty surgery. You appear to have a pleasing upper eyelid appearance. Generally, I do not recommend injectable fillers in the upper eyelid area since the skin is so thin. Your brows appear to be at a good height. I understand that you're dissatisfied. There is simply no substitute for a detailed, in person assessment. You need to meet with a highly qualified surgeon who is in the business of revising eyelid surgery. What I notice when I look at this photo is that you have upper eyelid ptosis. However, during a consultation, a lot of listening is essential to understand what is bothering you. I would not get a "botox brow lift." You are just rolling the dice. That service will not fix your upper eyelid ptosis and may very well complicate your situation. Please don't dig a deeper hole. Yes there is help. Your eyes can be your best feature.

I do think you might consider a temporal browlift. It's a great procedure. I actually think most patients who come in for upper blepharoplasty probably need a browlift more than blepharoplasty.

It appears that the lateral incisions from your blepharoplasty did not take in enough skin. You can have this excised revising the lateral portion of the blepharoplasty.

If you are smiling in the photo, things look pretty normal, if not, a small amount of additional skin could be taken along the outside of the lids. A temporal lift could also be an option. I recommend you discuss with your surgeon and see what they recommend. A botox "brow lift" could also help. That's certainly easy to try.

Unfortunately a blepharoplasty does not optimally correct lateral hooding and one study indicates that blepharoplasty on average slightly lowers the brow which has been my personal explerience. A temporal browlift which can be done as an office procedure will give a long term solution, and can be done as an office procedure. In an analysis of my last 200 upper blepharoplasties I have performed temporal brow lift in approximately 2/3 of cases to address lateral hooding which is usually present. I have written about and lectured on this topic. The healing is rapid with about 10 days to return to work from an appearance point of view.

To make an assessment regarding the outcome of your procedure, we need to see a complete set of proper before and after pictures.
If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took.
It looks like you’re contracting your Orbicularis oculi muscle in the picture which is causing wrinkles to form.
Botox might help but in order to get an assessment we need much better information.
Get a complete set of proper before and after pictures and consider reposting with those.
