Great experience!! Dr. Hilinski is a great, kind, and skilled plastic surgeon. I looked everywhere for a surgeon that could help with my hypertrophic orbicularis oculi muscle, which causes creases under my eyes. After several consults and multiple surgeons turning me away as they weren't skilled in fixing that issue, I found Dr. Hilinski. He did an amazing job; the clinic staff was super nice, welcoming, and friendly! Highly recommend! :)
Had ear surgery (macrotia) back in March by John. Such a great, seamless, surreal experience. I travelled from the UK which turned out to be easier than expected. John and his staff were all super accommodating. Felt safe in their care and confident I'd have my expectations met. The surgery has certainly provided me with more confidence. Wish I done it sooner. Can't put a price on feeling more comfortable in your own skin. Very grateful.
I had no idea there was a surgeon out there that does ear reduction surgery, but I found John after searching online for otoplasty and found that he also does ear reduction. I decided that I was going to go for it, because after 37 years of living with large ears and the constant hazing and mocking growing up as well as even into adulthood, I wasn't going to hesitate regardless of the cost. I have always covered my ears with long hair the best I could, but it wasn't something I wanted to do the rest of my life. I can say that this was a really good experience for me, and the cost was very reasonable. John is one of the few (or maybe only ones?) that does ear reduction, so he is very experienced with this surgery, and I could tell on the day of surgery that he is very precise and meticulate.. he makes sure it is done right the first time and looks completely natural. He made me feel comfortable and gave me plenty of reassurance that he knows what he is doing and after seeing the results I would say he is definitely one of the best. I would highly recommend. Thank you John you gave me so much more confidence.
I am so pleased with my results. I had revision septo-rhinoplasty. My first surgery (by another surgeon left my nose scooped, irregular, and somewhat “done” looking which resulted in another 10+ years of not liking my nose. Dr. Hilinskys detailed website and impressive number of before and after’s led me to book a consult. Additionally, I sought out previous patients who shared very positive results. I must say; I almost chose another doctor because he seemed more personable during consult. However; the photos, website, testimonials and just my gut knew Dr. h was the one so thank goodness I listened to that. Anyways, results!!!! I was so nervous to fix my “done nose” for it to just look even more done. However; my results are SO natural!!! My nose is beyond my expectations and feels like ME. My breathing is improved, he fixed my inverted V deformity which made my nose have weird shadows and scooped look, he reduced the projection of my tip and columma hang, he straightened my nose, and created my symmetry. Despite ALL of these changes, you can hardly tell my nose looks different, it just looks so dang good and normal :) I used to hate most angles of my face and didn’t realize how much my nose impacted those angles bc now I love most angles of my face. I’m so happy. He’s truly a magician and very humble. Happy to send photos or talk through anything with anyone. I’d certainly like to pay it forward as a few people helped me with my decision by sharing their experience with me. Oh! Also, no scar!! All stitches are inside my nose! I’m 3 weeks post op.
I recommend rhinoplasty by Dr. Hilinski, in San Diego. I am 49, live in Las Vegas, and had my rhinoplasty November 2021. I have satisfaction every time I look in the mirror, and I’m happy. I just didn’t feel right going anywhere else when I did my research. My expectations were exceeded, the work was a definite improvement, and that translates into tremendous value. I also got the impression that the staff was very well selected because of how intelligent and diligent each were, and everything was just right.
I don't know where to start from...All my life I was insecure about the shape and the size of my ears. I am 32 year old female that never did a ponytail in public. I grew up hiding my ears with long hair and never though that I would have my hair up front of others. Dr. Hilinski changed my life when I found him. I never knew that I could get a surgery to reshape my ears. Like who knew?! lol... I did an extensive amount of research then I found Dr. John Hilinski. I think he might be the BEST doctor out there that who actually does macrotia procedure! If you're thinking that you want to your ear reshape, do yourself a favor and get in touch with the DR. Hilinski. I promise your confidence will boost up and you will be happy forever!
My instincts told me to go with hilinski and my instincts were spot on. He did a revision for me and I couldn't be happier with my results! He really heard me and he communicated by email before my surgery which is exceptional service. He gave me the miracle I needed this year. He is a gifted surgeon and his staff amazed me. I will be back in the future for more precedures.
Dr. Hilinski is an exceptional cosmetic surgeon and I have complete confidence in his ability to give his patients the best care possible. He performed rhinoplasty, blepharoplasty, and laser treatments for me and I am 100% satisfied with the results. De Hilinski was able to give me a more youthful appearance but also retain a natural look. His staf is friendly and professional and very personable. I highly recommend Dr. Hilinski for any cosmetic facial surgery!
I'm elated I had Dr. Hilinski do my revision rhinoplasty. He is not only a highly trained doctor, but his kind demeanor & his ability to listen to my concerns put me at ease. He's an incredible doctor & all his staff are fantastic too. His office is impeccable & it's great because he does his surgery in his building so you don't get tacked on an extra bill for the surgery center. This wasn't my first or second rodeo so I consulted with other top doctors in Beverly Hills & another out of state. I'm grateful to Dr. Hilinski for my nose.
Sought out a revision rhinoplasty for YEARS, and finally settled on Dr. Hilinski. Couldn't be happier with my decision. He improved by nose, both aesthetically and functionally, and it looks totally natural. I made the mistake in 2010 (I was only 18 years old) of undergoing a primary rhinoplasty with a plastic surgeon, who happened to be a family contact. Although I received advice from someone who I trusted, the result was not great, and left my nose crooked and deviated. Almost 10 years later, and multiple visits to different surgeons, I concluded that Dr. Hilinski was the man for the job. I'm ~2 weeks out of surgery, and couldn't be happier. There is still a bit of swelling, obviously, but the positive results are evident. If you are interested in pursuing a primary or revision rhinoplasty, look no further, and go to Dr. Hilinski. Updated on 11 May 2021: Posting various before and after pictures. There is still a bit of swelling in the bridge and tip, but you can see how much more natural the tip looks, that the bridge looks stronger/more 'masculine,' and, most importantly, I can breathe much better out of my left nostril. There is still some fullness on the left side of my nose (right side of the picture), which has been there since my primary rhinoplasty. Nothing is perfect - I cannot stress this enough; expecting perfection is insanity - but I'm so much happier with the improvements. Updated on 11 May 2021: Below are before/after pictures. You can see that my tip is improved and looks much more natural (it was pointed and asymmetrical before), my bridge is stronger/more "masculine," and most importantly, I can breathe much better out of my left nostril. There's still some swelling, particularly in the tip and on the bridge, but that's expected. There's still some fullness on the left side of my nose, which will always remain to a certain extent, as this stems from my primary rhinoplasty, but I'm really, really happy with the improvements. Perfection isn't realistic, so please DO NOT expect perfection, as you will only drive yourself mad. Updated on 22 Sep 2023: I want to say that my nose was absolutely improved after my revision rhinplasty with Dr. Hilinski. Specifically, my bridge and tip were massive improvements. Now that I am over two (2) plus years post-surgery, the only thing that bothers me is that my nose is still asymmetrical and off-center. I do wish this would have been fixed, but I know that there is no such thing as perfection. That said, I did want to let others know on here. Updated on 22 Sep 2023: Adding some current pictures. Again, overall, very happy with my bridge and the tip. The only "negative" is that I wish the symmetry would have been fixed, as my nose is still a bit off-center. I don't want to be nit-picky, and I hope it doesn't across as such. However, I did recently have a follow-up, virtual consultationw with Dr. Hilinski, and he even agreed that another revision would be needed if I wanted to "correct" the symmetry, but I'm not really sure it's worth it, from a risk and/or financial perspective. Updated on 22 Sep 2023: Adding a few additional pictures. These pictures are from 09/22/2023. I’m happy to answer any questions. I know ow difficult and anxiety inducing the decision to move forward with surgery can be, especially a revision, so hopefully this helps, even a little. Wishing everyone good health, and remember to love ourselves!
It’s absolutely reasonable to want to combine rhinoplasty, a lip lift, and lower lid blepharoplasty with fat repositioning to reduce anesthesia exposure and recovery downtime. Many facial plastic surgeons—particularly those who are double board-certified—routinely perform this type of combination surgery, especially if they focus exclusively on facial procedures. The key is choosing someone who is highly experienced in all three, not just one or two. A consultation with a specialist will help determine if you’re a good candidate for this 3-in-1 approach and ensure it’s done safely without compromising your results.
Thanks for your question. A deviated septum can cause nasal obstruction, but correcting it with septoplasty alone typically won’t straighten the outward appearance of a crooked nose. If your main concern is the cosmetic shape, especially since you've felt insecure about it for years, rhinoplasty would likely be the more appropriate option—possibly in combination with septoplasty if breathing issues are also a factor. Adding a chin implant can be a great way to further balance facial proportions, especially if you're looking to enhance overall profile harmony. I’d recommend consulting with a board-certified facial plastic surgeon to review your goals and examine your facial structure in detail.
Thanks for your thoughtful question. A subtle midline indentation or cleavage in the nasal tip can indeed contribute to a feeling that the nose isn’t ideally suited to your face. While most rhinoplasty cases online highlight dramatic changes like large bumps or humps, more refined aesthetic concerns like yours can also be addressed with surgery. A well-planned rhinoplasty could help smooth the contour and enhance nasal definition, improving overall facial harmony. You’re right that filler in this area is risky due to the vascular anatomy, and not all providers offer it for that reason. Consulting with a board-certified facial plastic surgeon who has experience with subtle nasal reshaping would be the safest and most effective next step.
Thanks for the detailed question — this is a common and very valid concern following orthognathic (double jaw) surgery. You're absolutely not alone in noticing changes to the nose, philtrum, and overall facial balance. Here’s a breakdown of what’s likely going on: Why Your Nose Changed After Jaw Surgery: Widening of the Nose – Advancing the maxilla (upper jaw) tends to widen the alar base (the outer edges of the nostrils). This is particularly noticeable if no alar base cinch suture was done during surgery. Nasal Tip Rotation (Upturned Nose) – When the upper jaw moves forward and/or upward, the nasal spine and septum are repositioned. This can cause the tip to rotate upward, increasing nostril show. Longer-Looking Philtrum – This is partly an optical illusion. With the tip rotated up and nostrils more exposed, it can visually elongate the space between the base of the nose and the upper lip — even if the actual philtrum length hasn’t changed drastically. So, What Are Your Options? 1. Rhinoplasty (Highly Considered in Your Case) Since your nose was small to begin with and now appears wider and more rotated, a conservative, finesse rhinoplasty could: Narrow the base (alar base reduction) Derotate the tip slightly Reduce nostril show Restore facial balance, especially in profile and frontal views This type of revision is commonly done post-jaw surgery to “fine-tune” proportions once the skeletal foundation has healed. Lip Lift? You’re absolutely right to be cautious. A lip lift in the context of a short nose or upturned nasal tip can create awkward angles or make the nose look even more prominent. It can also worsen nostril show. Unless the philtrum is truly long on precise facial analysis (which would involve front and lateral view measurements), a lip lift might not help — and could potentially disrupt harmony further. Let me know if you'd like a list of specific aesthetic goals to bring to a consult.
Yes is the short answer. This type of deformity is fixed quite regularly with revision rhinoplasty techniques that include use of cartilage grafts to help 'efface' the depressed area. Make sure you go to a board certified expert with dedicated experience in revision rhinoplasty.
