25 years old - Will fat repositioning work for under eye bags? (Photo)

Hello, II am wondering what these bags under my eyes are and if fat repositioning would work to get rid of them. Ive had them since i was a young child. Ive slept enough, tale vitamins and have tried a variett of eye creams with no luck. Im 25 years old.

These bags correspond to the natural fat pads that live just underneath the eye. In some people the septum which holds them in place can weaken leading to a prolapse of fat that has the appearance of bags. It can also be discolored due to the tension on the skin. Some people are genetically predisposed to these bags. The treatment for this type of issue is a lower blepharoplasty with or without fat repositioning. In my experience, patients do quite well with this procedure and from that photo I think you have the potential to have a great result. I think your best option would be to schedule a consult with a plastic surgeon or oculoplastic surgeon that specializes in eyelid surgery who can discuss the nuances of surgery.

Bags that have been present since childhood and do not respond to sleep, vitamins or creams are almost always anatomic. The fat pads under the eyes are pushing forward, often with a hollow beneath them, and no cream can reach this because it is a structural issue.
At your age the best solution is a lower blepharoplasty with fat repositioning. This fat is valuable tissue, and at 25 removing it is not a good idea because it can leave a hollow and aged look years later. Instead the fat is preserved and moved into the hollow area to create a smooth transition, and the result is typically long lasting. I would not recommend filler here: it may look fine at first, but in this area it tends to swell and shift over time and in the long run you would most likely regret it.

Yes, those bags are fat from your eyeballs that are pushing through the lower eyelids. It is likely because you don't have much cheek support, which can be genetic, and that is causing them to appear more prominent/ahead of the cheek sooner in life. A lower bleph will fix this.

Hi Straightforward607805, thank you for your question and photo. When under-eye fullness has been there since childhood, it usually behaves differently from the bags that appear with age. From your description and photo, the likely candidates are a genuinely prominent fat compartment, a deep tear trough where the cheek meets the lid, or simply the natural shape of your midface. Creams and sleep will not change any of those, so please do not feel you have failed with them. Fat repositioning can work well when there is true fat prominence with a hollow beneath it, because it smooths the junction using your own fat rather than removing it. At 25, with thin under-eye skin, restraint matters: removing fat too early can create hollowness later in life. An in-person assessment with an oculoplastic-trained surgeon, looking at the lid, cheek support and skin thickness together, is the only way to know which of these applies to you.
