30, Female – Combining Arm Lipo/Fat Grafting & Upper Bleph?

Hi everyone, I’m a healthy 30-year-old woman considering subtle facial rejuvenation with long-lasting results rather than repeated fillers. I’m thinking about combining: *1 Arm liposuction *2 Fat grafting to the tear troughs, under-eye/midface transition, possibly the paranasal area, and lips *3 A conservative upper blepharoplasty A few questions: 1. Is AirSculpt a good option for harvesting fat for facial grafting, or is traditional liposuction generally preferred? 2. Is it reasonable to combine these procedures in one surgery? 3.Would facial fat grafting and/or an upper bleph make a facelift 15–20 years from now more difficult?

Taking your questions in order. First, the harvesting device matters less than gentle, low-pressure collection and careful processing of the fat; standard syringe or low-suction liposuction is widely used for facial grafting. Second, combining an arm harvest, facial fat grafting and a conservative upper blepharoplasty in one sitting is commonly done in healthy patients, although total operative time and anesthesia deserve thought. Third, conservative grafting and an upper lid procedure do not generally make a later facelift more difficult. Overfilling the face is the more common problem, and lips tend to keep fat less predictably than the tear trough or midface. An in-person evaluation with a surgeon experienced in facial fat grafting can help you decide how much is enough, and where.

These are excellent questions. I think combining these procedures is reasonable, but there's an important distinction between liposuction for body contouring and harvesting fat specifically for facial transfer.

I would NEVER use fat obtained through conventional machine-assisted liposuction or AirSculpt for delicate facial grafting. Facial fat transfer requires a different approach, using very small, delicate harvesting cannulas and gentle, controlled negative pressure. This helps preserve fat cell viability, minimizes tissue trauma, and may reduce the likelihood of irregularities and lumps. You simply cannot assume that fat harvested for conventional liposuction is suitable for placement around the eyes.

Yes, arm liposuction, facial fat transfer, and a conservative upper blepharoplasty can generally be combined in one operation, provided the patient is an appropriate candidate. I would harvest the facial fat separately, using techniques specifically intended to preserve graft quality.

And no, properly performed facial fat transfer or a conservative upper blepharoplasty should not ordinarily complicate a facelift 15–20 years later.

Ultimately, facial fat transfer is extremely technique-dependent. The harvesting instruments, processing methods, anatomical depth, and quantity of fat placed all matter. This is particularly true around the eyes, where even a small irregularity can be noticeable. The goal is natural, lasting improvement without creating an overfilled appearance.

Thanks for having such a detailed question. I typically recommend traditional power assisted liposuction when performing fat grafting. With newer technologies that people are using in combination with liposuction, there isn't data on the quality of fat for fat grafting. The goal is to minimize trauma to the fat cells to increase viability. 
It is definitely reasonable to combine these procedures and will allow you to have one recovery/downtime. 
Finally, facial fat grafting and/or a blephroplasty should not change the ability to have a facelift in the future. Most surgeons who performed facelifts regularly have no problem operating in a prior surgical field.

Hi - to answer your questions:
1. Most surgeons would prefer low-pressure, manual handheld liposuction as an atraumatic liposuction technique for harvesting fat grafts for the face, in order to maximize viability of the fat cells. However, after the fat grafts are harvested, the rest of the arm liposuction could certainly be performed with AirSculpt
2. It is reasonable to combine these procedures, provided that you are healthy enough to undergo general anesthesia for the projected time in the operating room
3. Facial fat grafting and/or upper blepharoplasty should not make a facelift in the future more difficult
