Lumpiness - Can fat transfer be removed from forehead 18 years post?

Question

Had a fat transfer to forehead 18 years ago which should never have been done. Some died and left me with some lumpness. Is it possible to remove this or at least lessen with a hairline browlift procedure?

Lumpiness is one of the frequent problems we encounter after fat transfer and it happens in the best hands. Correction involves selective liposuction with small cannula, injection of small dose of corticosteroids, detect excision/removal of the lump. Without good photos and in person evaluation it would be difficult to suggest what would be the best procedure for you.

Lumpiness this old can usually be improved, though "fully removed" is harder to promise since the fat has integrated with the tissue over 18 years. Because it is your own fat it cannot be dissolved, but firm lumps can often be flattened with careful subcision, conservative contouring/liposuction of the area, or a steroid injection for any scar component. A hairline (pretrichial) browlift gives direct surgical access to smooth or excise the bumps while also lifting the brow, so it is a reasonable approach if you would benefit from both. The best choice depends on how deep and firm the lumps are, which an exam and possibly an ultrasound would clarify.
