I am 23 years old and have always had these mounds which are really upsetting me. I was told by one doctor that it isn’t fat and two others said it is fat so I am unsure what to do at this point.

- volume loss in apple cheeks area - sagging of middle face area - excessive fat in area of cheeks and jaw part 
In this case, complex treatment might be the option : liposuction with fat pad reposition in area of apple cheeks, liposuction in jaw part including side cheeks. 
If 
1) There is sagging because of skin laxity, then some simple lifting treatment like threads or some laser in future, might help. 
2) Face shape is uneven, other part might be filled with fat to make it more smooth ( hollowed temples etc)

Do not let someone liposuction near your mouth and jowls. Even if this is far, and liposuction works temporarily, it will set you up for sagging skin in the future.

It is very likely that these are superficial fat deposits. Liposuction with a small cannula would be my first choice. Good luck.

Do not do any liposuction of try to remove facial fat for this will age you when you are older. I would suggest micro-needling with radiofrequency to help tighten your skin.
