I had a rhinoplasty done exactly 12 months ago and feel like my tip dropped and is pointy after consulting with my dr he advise he would lift up my tip with local anesthesia but not exactly sure if that will help with my situation or if a revision will be needed instead

If you are unhappy with the results a revision procedure will be needed. During your revisionary surgery your surgeon can address your concerns. The decision of undergoing surgery is a personal decision and of course as surgeons we can only make suggestions.

I understand going under a second surgery is difficult so please make sure to consult with a qualified surgeon. During your consultation you should go over photo renderings and explain to the surgeon exactly what your concerns are. He/she should be able to tell you what is and what is not achievable.

It does appear that your nose Slightly drooped after the surgery which is normal. There Appears to be small residual dorsal hump present just above the tip and hanging columella can be addressed with a revision rhinoplasty performed under  general anesthesia. This is not an office procedure.
