I had a rhinoplasty done exactly 12 months ago and feel like my tip dropped and is pointy after consulting with my dr he advise he would lift up my tip with local anesthesia but not exactly sure if that will help with my situation or if a revision will be needed instead

Due to previous surgeries and multiple trauma, it is possible to give you some improvement as long as you have realistic expectations. Your nasal bones appear to be excessively wide and can be narrowed with medial and low lateral osteotomies.
Based on your photos, it is not clear if you would need revision rhinoplasty - it's too soon to tell. Your nostril asymmetry is minimal and may improve, and so may your bridge asymmetry. When you see your surgeon for your 6-month follow up, get a copy of your B&A photos and consider re-asking...
