Where exactly are you trying to get rid of? You don't realy have much of a sill left. If you're talking about the flaring next to your columella, that's from your medial crural footplates which would need to be addressed in a different fashion. Why are you worried about your sill width?
Based on your photo, you appear to have prominent foot plates of your medial crural cartilages. This is right next to your nasal sills, so I can understand your labeling. When these foot plates are prominent, as in your situation, they may be sutured together to be minimized during rhinoplasty. If you would consider rhinoplasty for a narrower nasal appearance, this is a maneuver that could be considered. We have never had the opportunity to address a concern like this as an isolated concern, more commonly, it's part of an entire rhinoplasty procedure. If you would consider rhinoplasty surgery, when consulting with an expert, this is an excellent source of dissatisfaction to bring to the attention of your prospective surgeon. If you get rid of the nostril sill your nostrils will be so small that you might not be able to breathe. A standard alar wedge resection would be a better option
Hello, The area near your sill that seems to be creating the issue is not exactly the sill itself. It seems that the base of the tip cartilages ("medial crural footplate") is flaring out and creating the fullness. The contrast in skin tone is also making it more visible. The way light bounces from more prominent surfaces can also contribute. The medial crural footplates can be addressed in a few ways, each producing a slightly different overall effect on the surroundings as they are the base support for the tip cartilages. If you suture them together in your case, you will get more tip projection. If you remove them completely, you will lose some tip support. The best option would be to find someone that truly specializes in rhinoplasty and will sit down to explain all these factors, as well as the options and their ramifications. Good luck!
Your previous surgery makes the situation difficult and complex.The incisions are quite visible, and whenever tapering tissue is removed, there can be a mismatch in volumes at the incision line.I would not remove any more tissue in this area.Best,Mats Hagstrom, MD

Rhinoplasty surgery tends to lead to a lot of bruising and swelling around the eyes. In that regard, I would suggest waiting several months after Rhinoplasty to have your under eyes addressed. I hope this helps! Sincerely, Dr Joseph
