Hi there, This is a tricky issue -- it seems as though your skin scar is stuck directly down to the muscle. Is it painful? There are a couple options-- if you really want to avoid another surgery you could try subcision and fat grafting but this would require at least 2-3 procedures to make a significant difference. You could also try an in-office scar revision with a layer closure. That is probably best done with you under sedation but could also potentially be done awake depending on your tolerance to that. Hello, i had my breast implants removed on August 22nd, partial capsulectomy, textured implants, strattice mesh. I have no breast tissue at the mid/bottom portion of my breasts. I’ve developed an adhesion behind the left nipple (attached to the chest wall with a thick tether), and the right nipple is folded over. My question is: is a whole surgery needed or could this be resolved with a scar excision..… some sort of office procedure ? I’ve had 4 breast surgeries already and would love to avoid another.

Hi there, I order to lift your breasts in a meaningful way, you would need to have a peri-areolar scar, a scar from your areola down to the base of the breast, and also a bit along the base of the breast. Many people think of this like: trading scars for an improvement in shape. It looks like you have a decent amount of breast volume though, so you certainly don't need an implant. I was wondering if I qualify for a breast lift and if I would need inplants also or if I could have nice shape with minimal scarring. if not what procedures would I need?

Your age alone doesn't make you not a candidate for surgery. It increases some peri-operative risks but, in my opinion, as a sole factor doesn't preclude surgery. Happy to help however I can. Dr. Safa Sharabi

I am 66 years old, 5'2", weigh 100 lbs. I work out five days a week, I'm on natural hormone replacement ,therapy but have loose skin above my c-section, had one birth when I was 41. I am wondering if I am too old to have a tummy tuck?? I went for a consult and the doctor said he doesn't operate on women my age because he can't guarantee elasticity of the skin. He said I have diastasis and umbilical hernia and all the workout in the world will not make me narrower. I am in great shape everywhere else. HI spent my 40s and 50s caring for my son and now I feel great, but am self conscious about my middle. Am I wasting my time? I take no medications, very healthy.

Hi there! It's your body and if you don't want a lift, you don't have to get one! I've had plenty of women tell me they would rather have a bit of a droop of their natural breast tissue over the implant instead of a scar and I think that's totally reasonable. I would recommend going sub-muscular, since your upper tissue thickness is quite thin. The only thing left would be your choice of Motiva vs smooth implants and your choice for size. TL;DR No hope for implants w/o lift? 5'8 - 140lbs (had a 60lb loss in 2020, maintained since) Breastfed for 2 years (2016-2018) I had a virtual consult with CEC in Costa Rica because I've heard such good things about the Preservé method and the minimal scarring from the small incision. They suggested a lift as well, but I'm not interested, and they wouldn't know for sure until I was there in person. I don't want them perfect, just fuller in a non-deformed way. I actually like the teardrop type shape, but mine look like empty pockets. Obviously I'm not the expert, and would love to pulse check. Would most docs say the same?

It's impossible to tell from just the photo (one would need to be able to pinch the skin to know for sure), but it's likely that at least some of that residual areola skin could be removed, thereby minimizing the appearance. And yes, local anesthetic would be ideal for this type of procedure.

I underwent a breast lift with implants in October 2020. Since then, I have had this ongoing insecurity and concern regarding leftover areolar tissue on both breasts. The left side where the tissue extends over a larger area beneath the primary areola is the one that bothers me the most. What doesn’t help is the fact the my primary areola are already larger than I would prefer. I would like to know whether it would be possible to have this excess tissue removed using local anesthesia.

Hi there! I agree that you have a decent amount of existing breast tissue. A Motiva Mini, at 11.5 cm base width, would give you 245 cc of volume. That's going to increase your projection by about 3 cm and provide you with some more upper pole fullness. I think you'll have a great result! Sincerely, Dr. Safa Sharabi

Hi I’m 27 years old 5’5 122 pounds I want to get a breast augmentation. Skin stretch 2. PCSEF 60%. Base width 11.5 at most. I want something noticeable but natural. Not sure what size/profile to go with. I don’t want to look BIG. I’m a pretty fit person I have a good amount of breast tissue, I’m a small B cup. I do want some upper pole volume and overall breast fullness but I don’t want to look overdone. Any advice would help.
