Hi, I have what I think are flared or overarched nostrils. When I smile, this becomes more pronounced and the tip gets pulled down. Is this a tension nose, given the thinner upper lip? Could the tip pull and nostrils be fixed just by incision to lateral muscle at base of columella? I am happy to not alter the dorsal hump and tip. I also have a mildy deviated septum. Many thanks!

Answer: Tension nose Yes, this would be termed that in that with certain expressions the columella may appeared pulled. The correction is release of the muscles attached to the columella as well as reduction of the caudal septum and interdomal sutures to correct the relationship between the medial and lateral crura.

Answer: Rhinoplasty Yes that would be a tension nose. Your tip projects forward and it’s like pitching a tent on the tip of the nose. Correcting it is pretty basic fir an experienced surgeon. But you need a full rhinoplasty including raising the tip and lowering the bridge. I think this is best done as a closed rhinoplasty. The tendency for some surgeons with a nose like this is to leave it too long
