Hi, I have what I think are flared or overarched nostrils. When I smile, this becomes more pronounced and the tip gets pulled down. Is this a tension nose, given the thinner upper lip? Could the tip pull and nostrils be fixed just by incision to lateral muscle at base of columella? I am happy to not alter the dorsal hump and tip. I also have a mildy deviated septum. Many thanks!
Answer: Rhinoplasty Hello! Thank you for your question. From the photos you provided I can say that you have a serious hanging columella and dorsal hump. Also you might need external graft for your nostrils. It would be more convenient to decide with face-to-face consultation. Best Regards!

Answer: Things you should know about rhinoplasty Thank you for your question and for providing photos. During rhinoplasty, it is possible to narrow the nostrils and fix the drooping tip while not altering the rest of your nasal features. The septum can also be straightened to provide optimal function while creating a feminized result. Since rhinoplasty is one of the most complex of all plastic surgery procedures, requiring experience, skill, and even artistry to achieve an optimal outcome, I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. Working with someone who has expertise in rhinoplasty techniques is most likely to help you address your concerns and achieve the best results.

Answer: Indeed... A tension-nose or tension-lip is when you have a prominent anterior nasal spine that blunts the angle between your nose and lip. In your case, this seems to be associated with a long nasal appearance, a hanging columella, and a dorsal hump. You also appear to have this nasal skin which may be prone to show irregularities after rhinoplasty, compared to thicker skin types. Please consult with several reputable rhinoplasty specialists. Thanks! Dr Joseph

Answer: Tension nose Yes, this would be termed that in that with certain expressions the columella may appeared pulled. The correction is release of the muscles attached to the columella as well as reduction of the caudal septum and interdomal sutures to correct the relationship between the medial and lateral crura.

Answer: Rhinoplasty Yes that would be a tension nose. Your tip projects forward and it’s like pitching a tent on the tip of the nose. Correcting it is pretty basic fir an experienced surgeon. But you need a full rhinoplasty including raising the tip and lowering the bridge. I think this is best done as a closed rhinoplasty. The tendency for some surgeons with a nose like this is to leave it too long
