Surgeon said he'll remove nose filler during rhinoplasty - is this normal? Shouldn't the filler be dissolved before?

Hi @gaige.guillory! This is a preference of the surgeon. If a HA based dermal filler was used, Hylenex can be used to dissolve the filler. Another option would be to remove the residue of the dermal filler while in surgery. Best of luck!

HA fillers stay for longer than expected in the nose, because your nose is static: it doesn't move. We have been in this situation, and it's amazing how quickly hyaluronidase may work in some people - days not weeks. This, in my opinion, is the favorable way to evaluate for rhinoplasty. Thanks! Sometimes our patients do not say that they have had nasal fillings before. But when we open the nose, we see it. We can clean the filling and continue with the surgery. No problem. A filler melter can be made 30 minutes before the operation and the operation can be performed in this way.

Thank you for your question. If the filler is not dissolved prior to having rhinoplasty surgery, it's not a problem as it can be removed during the operation. In my practice, I use Juvederm, which takes about 9 to 12 months to dissolve completely. It's a personal choice to wait or to get it removed by your surgeon during your surgery. Generally it is better to dissolve the filler before surgery for better assessment of the nose shape. However, if the filler is not dissolved before, it wouldn't be a big issue as it can be removed during surgery.

If the filler you have made is Hyaluronic acid, dissolving can be tried before the surgery, but not all of them can always dissolve. Therefore, we can also clean the fillers during surgery. So, your surgery can be done and fillers can be cleaned without dissolving. In fact, the ideal is that a patient who is considering surgery should not have a nose filler application. Hyaluronic acid based fillers can be dissolved in the office through injection of hyaluronidase, an enzyme. If you've had other, non-dissolvable fillers placed this may not be an option.
If the filler can be dissolved, I would recommend having this dissolved completely so that you and your surgeon can fully assess the appearance of your nose in its non-augmented fashion. This will help you both have a true understanding of your unaltered nose for an accurate assessment as well as to compare to the postoperative results.
