For south Asian noses, must you project the tip to help reduce projection?

I like my height of my nose. But I often struggle with side profile, sometimes projection seems too much. I was given two morphs, firstly I asked for bump to be taken out, and do what works for my facial harmony. But it looked too small for my face. Then I asked to keep height of my bridge and see if you can reduce projection. Does any of these look like they balance my facial harmony from side ? I worry if I reduce my bridge, distance between my eyes will change front view.

Hello and thank you for your question.

Although an exam in person is ideal, from your photographs alone it appears that you can benefit from a subtle profile reduction and tip rhinoplasty where the nasal tip cartilage is modified for a more refined, feminine look.  In my hands, the tip refinement would involve tip deprojection with lateral crural steal secured to a columellar strut, and placement of a mastoid fascia tip graft, in addition to extended spreader grafts to maintain the tip at tension and extended alar contour grafts.  If your septum is deviated and you have breathing difficulty, a septoplasty may benefit you as well -- insurance can cover this and reduce the price of your surgery☝️

The next step for you is to pursue the right aesthetic plastic surgeon for your rhinoplasty.  In your rhinoplasty consultation, be sure your surgeon addresses all your aesthetic goals and concerns specifically, IN PERSON, and discusses the surgery details including the position and size of the incisions used (with actual photographs and not just digital animations), the risks and benefits of the procedure, recovery time, and post-operative course.  I also think a key component of your consultation is to establish a good connection with your plastic surgeon.  This is difficulty to do without seeing your surgeon in the office, in person.  It’s the connection that allows you to clearly define your desired nose outcome goals, the first step of this whole process. If you don’t feel you have a good connection with your plastic surgeon after your consultation, he/she may not be the best surgeon for you.

Hello and thank you for your question. Based on your photographs, you are a great candidate for rhinoplasty. Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, among other maneuvers.

I highly recommend that my patients focus much more on real longterm before and after photographs rather than 3D imaging.   I encounter so many patients in my practice who see me for revision rhinoplasty who previously had surgery elsewhere who feel like they were mislead into surgery by surgeons who relied heavily on 3D imaging without adequate real before and after pictures to back it up.  Photoshop is easy but real surgery is very different.

I always recommend that you carefully evaluate your surgeon’s online before and after gallery on their website for both quality and quantity of results. Make sure that there are hundreds of real before and after pictures which demonstrate long-term follow up results. If your surgeon is posting mostly on table results without real long-term follow up results, that is usually a major red flag.  If a surgeon is posting mostly just splint removal day videos without longterm follow-up pictures, that can also be a red flag.   Long-term results are much more meaningful than on table results or 1 week post-op results.   Selfie’s can also be fun to look at, but these can often have a filter and do not represent true longterm results because of this and are not as valuable as real longterm in-office before and after photographs on your surgeon’s website.

With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
