Hello I am 27 and I’ve always had an issue with eyebags. I’ve had tear trough filled before but not since 2019. I also feel my eyes are droopy. A previous surgeon told me to “fix” my eyes first. What would be the best thing to make me look more awake or is it my pigmentation. I feel my eyes are droopy and I’m not sure what could be done to “fix” them.

Answer: You need cantoplasty... When I see your picture, the first thing to fix is your lower eyelids. The shape of your lower eyelids are genetically low and we can raise them. With cantoplasty (other name of the surgrey is almond eye surgery) you can have a better look. For the upper eyelids I do not see a laxity from yhe photos but I must check them. Best regards...

Some patients by genetics or race may have this pigmentation around the eyes or it may be due to a health condition, such as brain injury. If what you want is to fix the shape of the eyes I suggest you consult an oculoplastic surgeon. An upper blepharoplasty at this time is not advisable because the skin has elasticity, it is very young. At the lower level I see pigmentation, I do not see bags. The problem can be treated non-surgically with hyaluronic acid "fine line" to improve the distance between the cheek and your eye and thus improve the appearance of your eyes. Best wishes!
