Hi, I’m 58 & had face, neck & eye surgery 3 years ago. Devastating results & I would appreciate advice about my eyes. Pre-surgery I had big, pretty eyes but droopy eyelids hence upper & lower bleph. Now I can’t close my right eye as much more skin removed from that eyelid; the right outer corner feels tight & the skin is puckered there & it waters a lot; I have lines running from the outer corner down my cheeks especially when I smile; I look injured, please help.
Answer: Blepharoplasty outcome To make an accurate assessment regarding the outcome of a plastic surgery procedure we generally need to see a complete set of proper before and after pictures. Overzealous skin removal of the upper eyelid is going to be difficult to improve upon. Your situation is complex, and I don’t think it’s appropriate to begin making an assessment or giving surgical recommendation based on such limited information. I suggest you consider having proper in person second opinion consultation. For a second opinion, consultations come prepared to bringing with you a complete set of before and after pictures and a copy of your operative report. These should be available from your current providers office. Provider selection is paramount for any surgical procedure, but recognize that revision surgery is generally far more complex and difficult than primary procedures. On top of this some truly talented providers may be reluctant to take on poorly done work done by others. Anyone who is willing to attempt revision surgery generally inherent all the bad outcomes done by previous providers. That does not mean there aren’t talented plastic surgeons available to help you. The following is a list of general recommendations to find the best provider. I believe principles are true for both Primary and revision procedures. To find the best provider, I suggest patient schedule multiple in person consultations with experienced plastic surgeons in their community. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before and after pictures of previous patients who have similar body or facial characteristics to your own. An experienced plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. An experienced provider should in fact have hundreds or preferably thousands of before and after pictures to choose from for commonly performed procedures. Being shown a handful of preselected images, representing the best results of a provider’s career is insufficient to get a clear understanding of what average results will look like in the hands of each provider. There is no correct number of consultations needed to find the best provider. The more consultations you scheduled the more likely you are to find the best provider for your needs. Take careful notes during each consultation, especially regarding the quantity and quality of before and after pictures. It’s not a bad idea to bring pictures of your own body/face to use as reference when reviewing before and after pictures. The biggest mistake patients make is scheduling only one consultation without properly vetting or comparing providers before scheduling surgery. Best, Mats Hagstrom, MD
