Thank you for your question. Yes your nipples are too high. The distance from your nipple to your breast fold is too long. Furthermore, your nipple should never be poking out of your bra or swimsuit. The correction is to decrease the distance from the nipple to the fold. Furthermore, your right breast is larger than your left breast. This could be revised to add that revision operation to fix the nipple height. Good luck.

Yes, you have bottoming out of your breasts with high positioned nipples. The distance from the inframammary fold to nipple is too long and this needs to be shortened to correct your breast shape and nipple position. It appears that your nipples are asymmetric as well, which can be fixed at the same time. Your breasts can be beautiful, just a few corrections!
Actually, your nipples are in a good position, but your breasts and implants have sagged. They can be raised and suppored with an internal bra.
Thank you for your question and photos. It does appear that the breasts have "bottomed out". This means the implants have descended below the inframammary fold causing the nipple to rotate upward. The distance from the nipple to the inframammary fold has increased. Unfortunately, there is no way short of surgery to fix this problem. This can be corrected by a lower pole mastopexy and a technique to support the lower pole of the breast to keep it from descending again ie: Galaflex or alloderm sling to maintain the lower pole- kinda like a hammock.This is not a simple operation so its very important to seek an experienced, board certified Plastic Surgeon to help you through this.Best wishes,Dr. Morrissey
Unfortunately you have experienced first hand the phenomenon of "bottoming out" If the surgeon makes the breasts look perfect on the operating table, then skin below the nipples stretches out making the nipples upturned. The only way to correct this without making scars above the nipples is to do another lift to push the breast tissue back up behind the nipples. Synthetic absorbable or biodegradable mesh or the patient's own dermis in the form of an "internal bra" can prevent or delay the re-bottoming out process.

How do you feel about your results? If you like them, then there’s no need for a lift. A lift is typically recommended when the nipples are located lower than the breast fold. However, it appears that your nipples are inline or perhaps just above, so a lift may not be necessary.
