It appears that your nipples are asymmetric as well, which can be fixed at the same time. Your breasts can be beautiful, just a few corrections!
It does appear that your implants are bottoming out, creating an extra long nipple to inframammary fold distance. This leads to the incision and nipple both riding up, while the implant is riding down toward the abdomen. The inframammary fold should therefore be restabilized and secured to correct this and provide implant support. If you have extra skin, this may also need to be tightened.
