Hello! I began with a deflated 36D cup and opted for 405cc smooth round Subfascial implants. My doctor said I was borderline for a lift, as I could hold a pencil under the fold but my nipple position was still good. I elected to forego the lift as I didn’t want to “mess with” my nipples. Based on this result, should I go back for a lift? Would a lift give a better aesthetic result?

A lift is typically recommended when the nipples are located lower than the breast fold. However, it appears that your nipples are inline or perhaps just above, so a lift may not be necessary.

Answer: Results You look good now with a very natural look. However, if you prefer, you could go back for a lift. With your anatomy, your breast naturally sit somewhat low on your chest and you will not be able to have high and perky breasts.

Answer: Explant with Bellesoma Method At size 36 D, you did not need implants but needed a lift. Now, you have large low breasts. At this time, I would recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired. The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering surgery, I would suggest you consult a board-certified plastic surgeon. Only after a thorough examination, you will get more information and recommendations.
