Your current implants are too large and heavy for your tissue and frame. You do need to go smaller and smaller implants will sit better in your pockets. The larger areolae can be made smaller and raised if needed.
The actual problem is that your implants are too large and heavy for your now stretched and thinned tissue. I would suggest going smaller. Mesh could be helpful. Just removing excess lower skin may not raise up the implant and skin does stretch.
