I got implants January 2022. I chose a modest size (270cc). I am happy with the size as I did not want anything huge for my petite body. A few months after surgery, my implants started slipping equally on both breasts. 2 years later, my nipples are pointing up and when I don't wear a bra, I can feel my implants curling at the bottom. In a bikini, I literally had to tape my nipples down. My dr suggested GalaFlex mesh. Has anyone had success with this?
Answer: Your doctor is correct Galaflex, the leading company in resorbable mesh, has changed the way surgeons feel about smooth implants. It has allowed us to fully let go of any want for textured devices, which present their own problems. Your case is ideal: good tissue characteristics, modest implant size relative to your anatomy, and symmetrical dropout that is not severe. The process involves removing the implants, placing sutures to 'close down the pocket' along the inframammary fold, replacing implants, and placing mesh along the repair. Discomfort will be mild.

Answer: Implants Even though your implants are not large and heavy, they are stretching and thinning your own tissue. The mesh will help give support when your pocket is lifted and tightened. You might even consider going down some in size, like 225 to 250, just to have lighter implants on your tissue.

Answer: Suggestion Without a physical exam, it is challenging to make a definitive recommendation. It appears that your nipple to fold distance is too long. This can be solved by performing an inferior horizontal resection of skin. Adding Galaflex will only strengthen the support for the implant over time. Adding the Galaflex scaffold, will not do anything to the where the nipples are pointing. To change nipple position, there must be some type of skin excision. I would find a surgeon certified by the American Board of Plastic Surgery; ideally, one with an aesthetic fellowship accredited by The Aesthetic Society. I would also review before/after photos, Google reviews, and talk to the surgeon directly if possible.

Answer: Implant slipped Dear Lauren08723,I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice. Answer: Revision Your implants are bottoming out and the weight of the implants is causing the skin to stretch and the implants will continue to migrate inferiorly. You will need the implants temporarily removed, plication of the capsules inferiorly and laterally and dissection of the upper portion of the capsules to allow the implants to be replaced higher on the chest wall. Best Wishes, Gary Horndeski, M.D.
