Question

- my nose needs to be made measurably longer as the nostrils and columella sit too high up (symptom of nose being over-rotated & lacking projection?) - the tip cartilages need to be moderately narrowed - the columella needs to be made longer & thicker - need projection added to the tip - need nostrils narrowed, preferably through the tentpole affect occurring secondary to tip grafts for projection as opposed to excision of tissue Are these goals achievable to the degree shown in photos?

Hey sept us the extension graft is a piece of cartilage that is attached to the bottom of your nasal septum. That serves to lower the columella and lead to a longer nasal appearance. A rhinoplasty can do multiple things. It can refine the tip as well as the bridge and change the width of the nose. You may also want to consider a lip lift. A lip lift does 4 things: shortens the distance between the nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.

Hello and thank you for your question. Based on your photographs, it is very difficult to offer specific advice as the photos are not clear. From your description you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, reshaping, and appropriately tensioning the cartilage in your lower nose. Cartilage grafts may also be used to help improve tip refinement. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person.

We typically make assessments of the nose in context to the rest of the face.
I suggest scheduling a few in person, consultations with plastic surgeons in your community to get a quality assessment and better understanding of treatment options.
I suggest patients avoid the temptation of making your own assessments and treatment plan.
