Your questions are exactly what is assessed and discussed at a consultation with a board-certified plastic surgeon. Unfortunately, that process cannot be safely or accurately bypassed. Implant selection and technique depends on a number of factors, all of which are personalized. This includes your specific measurements taken at your consultation along with other hands-on assessments of your tissues. Your questions are answered both generally and specifically to you. My suggestions is for you to have an in-person consultation with a board-certified plastic surgeon. Please be reassured that most patients find the consultation process to be both informative and enjoyable.  

It would be a mistake to try to select an implant on your own based upon internet research and recommendations from others. A multitude of variables will affect the outcome of your surgery. To obtain the proper answer to your question would require a formal consultation. A board certified plastic surgeon has devoted many years to develop the ability to understand these variables and provide the best guidance and recommendations for you. 

Specific recommendations cannot be made without an in person exam by a Board Certified Plastic Surgeon. That said, similar questions have been answered in principle and can be found on my website as a resource.  In general terms, sizing is done by the Tebbets High Five method taking into account tissue thickness, nipple to fold distance, breast width, skin laxity and current volume.For example, a lady with normal thickness, breast width about 13 cm, an A or small B cup, no stretching of N:IMF would get an implant in the 300-325 range under the muscle.  This would create upper breast fullness without excessive roundness and a C cup.

Implants in breast augmentation should be picked very carefully. This generally is not based on what the patient wants or the surgeon wants. This is determined by the patients body and their dimensions. If implants are selected based on your dimensions complications will be limited and the results will be optimized. In a patient like you I would recommend a sub-muscular position. This is the most common position and has a proven track record. In smooth implants this will also help decrease your risk of capsular contracture.

Hello and thank you for your question. There are several advantages
and disadvantages to over vs under the muscle.  Under the muscle has the
advantage of lower capsular contracture rates, less risk of rippling, and
better visualization with future mammograms.  Most patients say that under
the muscle also has a more natural look and feel.  The size, shape,
and profile of the implant is based on your desired breast
size/shape, your chest wall measurements, and soft tissue quality. 
This decision should be based on a detailed discussion with
equal input from both you and your surgeon.  This entire surgery can be performed with a
small incision technique.  Make sure you specifically look at before
and after pictures of real patients who have had this surgery performed by your
surgeon and evaluate their results.   The
most important aspect is to find a surgeon you are comfortable with. I
recommend that you seek consultation with a qualified board-certified plastic
surgeon who can evaluate you in person.
