Im1year post op from chin liposuction and unfortunately my surgeon did not do a great job. He was very aggressive and removed too much fat leaving me with neck bands showing -noticeable when I look up or make certain facial expressions. Is there a way to correct this with fat grafting? Where do I go to find a reputable surgeon to correct this? Please help!
Answer: Tread Lightly Hello, hope you are well. As you learned, there is a 'sweet spot' in submental adiposity. Fat can be removed to a point, but can skeletonize deep structures if removed too aggressively. Conversely, fat can be added by grafting, but one needs to tread lightly, and it can be a challenge to camouflage platysma bands without going to far or potentially causing other problems. The main issue with fat grafting is that there is always a degree of uncertainty with how much of the graft will survive and if it will do so evenly, and so on. Sometimes multiple treatments are required. Alternatively, you could try botulinum toxin treatments to the platysma which can be very effective in eliminating or reducing the visibility of bands. Or a surgical platysmaplasty (via a small incision under the chin) would be a reasonable approach to eliminate the banding. A robust HA filler can be used to lift and pull the bands laterally to smooth them and a low viscosity, cohesive, malleable HA filler can be used in boluses in the midline for further smoothing and the promotion of new collagen and elastic fiber synthesis. I gave up fat transfer many years ago. For one thing, the procedure is relatively expensive. For another, it is a two step, minimally invasive (means expect downtime and swelling) and usually requires the use of larger cannulas. For another, despite promises to the contrary, much of the transplanted fat, separated from its original blood supply does not survive in the new location, And finally, fat retains the characterisiics and properties of the area from which it is taken. So, if it is taken from the buttocks, thighs and belly, when these areas develop midriff bulge, so, too, may the fat transplanted into the neck. Answer: Liposuction results Delivering consistent quality, liposuction results is more difficult than most people realize. For this reason, I recommend patients be very selective about using providers. I recommend patients have multiple in person consultations before scheduling surgery. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients in the past. And experience provider should have no difficulty showing you the before and after pictures of at least 50 previous patients. Liposuction should be viewed as a permanent and irreversible procedure. Collecting poor outcomes is difficult and grafting fat may be an option, but without having insufficient layer of fat to serve as host tissue, grafted fat is unlikely to survive well. This is a difficult situation to improve upon. If you opt for fat grafting, expect improvements to come after multiple sessions. Revision Work is many times more difficult than Primary and Liposuction. The importance of finding is officially talented provider is more important for revision work then Primary procedures. For individuals who opt for fat grafting you need to not only vet providers for Liposuction, but also for fat grafting skills. The process for finding providers for a vision work it’s more or less the same as finding the best providers for primary procedures with the exception that you also need to confirm, they have experience with fat grafting. Restoration to normal may not be possible, but some improvements may be possible, especially if you’re willing to undergo a few sessions of revision work in the hands of someone with sufficient skill and experience.
