2 very different consultations, which approach should I go with?

A consultation is key to know for sure showing just the nose area and giving you recommendations just based on them is not posible, because the nose makes part of a whole. From the little I can appreciate I suggest a rhinoplasty to modify the nose and make the nose more defined and harmonious with over all face. When it comes to the question of what changes can be made during a nose surgery, there's no one-size fits all approach. The goals of a nose surgery are individualized according to the characteristics of each patient. When reshaping the nasal structures, I suggest one or more of these procedures be performed: Reducing the length of the facial tip, Straightening out bumps on bridge; reducing length and size while correcting breathing problems. Thank you for your question. You are an excellent candidate for rhinoplasty, I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Most plastic surgeons like to perform lip lift procedures at a separate time. Best wishes to you.

I think the bridge has to be lowered to get the effect you want. A tip graft alone will just look odd.

Based on the limited photographs, you're going to need to undergo a full rhinoplasty to ensure all components of the nose are balanced with themselves, and the entire new nose balances with your facial features in the three-dimensional fashion. A closed rhinoplasty approach can accomplish shaving down the dorsal hump, and narrowing the lower part of bridge line from the frontal view. An alar plasty can accomplish narrowing wide nostrils

From your photos, I note that you demonstrate a high nasal bridge and a relatively broad nasal base. There appears to be a lack of nasal tip support as well.
A full rhinoplasty would lower and narrow the nasal bridge and a cartilage graft will add tip support.
Anything less will not suffice.
