Excision of dog ears in local anesthesia is very simple. Extensive liposuction of the abdomen, flanks, and lower back, possibly extended TT with lipo, would be options to consider. Hope this helps.

You have a very nice result. "Dog ear" revision is very simple and includes extension of the scar laterally by a 2-3 inches which will not be visible and not a big deal. It can be done under local, and cost depends. If you go to the original surgeon, they will likely not charge you a surgeon's fee (up to $2,500 depending), and only charge you for hard costs (facility, nursing, and supplies). Dog ear removal without tummy tuck revision is possible with tummy and sides liposuction.
My recommendation would be scar revision for the current tummy scar in order to take it lower than its current plane + extending the scars to the sides to remove the excess skin on the sides and the dog ear + liposuction, these procedures would provide the best result and you'll have a really good form

Yes you can elect to simply have the excess skin at the end of your incision addressed. However it appears from your photos that you would have a nice improvement in your results if you added some liposuction for further contouring. This can be done at the same time as addressing the dog ears.

Yes, you can definitely just have the dogears removed. But, in my opinion, as long as you're there doing that, I would recommend lowering your scar at the same time so that it is better hidden

If it’s just a dog gear, you’re trying to fix then just fix the dog ears.
If you don’t like the step off at your incision line, then more work will be needed.
