One potential option is to focus on pectoralis bulking exercises to provide more definition to the pectoralis. This may be sufficient to prevent any further surgery. Another option is to revise the scars but this is complicated as surgery always leads to more scarring. I would have a conversation with your surgeon.

This is a common complaint that I receive from patients who have been operated on for gynecomastia with gland removal that really needed liposuction. There is a difference between true gland hypertrophy (overgrowth) and excess fatty tissue which obscures chest definition. When gland is removed in someone who only has excess fatty tissue, indentations may form and seem irregular. It is important to consult with an expert in these surgeries as this is a difficult problem to correct---albeit not impossible. Fat transfer works well in these instances to even out the contour irregularities from fat from elsewhere.

It's always unfortunate when the patient isn't happy with their results. You would most likely need fat grafting to your chest muscles to achieve a more defined look. Fat grafting to the area under your scars may help as well. If going back to your original surgeon is not an option for you make sure to find a surgeon who help you reach your goals and decrease the risk of further revisionary surgery.

I'm sorry to hear you are not happy with your results.
When I look at your after photos, I notice that the area under the areola looks depressed. This could be due to scar tissue formation or perhaps the amount of tissue that was removed was more in those areas.
Without examining you in person my recommendations can't be exact, however I do suggest that adding some volume to the area with a fat grafting procedure can soften the appearance, making it look less severe. I recommend you see your surgeon again in follow up and look at the before and after photos with your surgeon.

I agree with your assessment.
I’m guessing too much tissue was removed with the open glandular excision and or the tissue was removed in the wrong place.
Your chest shows an indentation at the base of your nipple which shouldn’t be there.
Gynecomastia surgery can be a bit tricky but you were a pretty straightforward good candidate for the procedure and expecting a better result is a A realistic way of seeing it.
I’m curious if your revision surgery made things better or worse?
Gynecomastia that has been left under treated it’s pretty straightforward to improve upon but once someone has been over treated making things look better becomes very difficult.
My best advice is is to recognize the importance of provider selection and properly vet plastic surgeons before having any permanent procedure.
Finding the right provider is easier said than done.
Personally I recommend patients start by having multiple in person consultations. During consultations ask each provider to show their entire collection of before and after pictures.
Confirm your provider is board-certified by the American Board of plastic surgery.
Being shown a handful of selected pictures which probably represent the best results of the providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider. This is why patients should ask providers to open up their portfolio and show their entire collection of before and after pictures or at least as many as they are willing to. an experience provider should have no difficulty showing you at least 50 sets of before and after pictures of common procedures. I would call gynecomastia a fairly common procedure. Highly experience surgeons will have hundreds or possibly 1000‘s before and after pictures for each procedure.
You should also ask the provider to show pictures of patients with very similar body characteristics to your own. It’s also fair game to ask providers to show examples of excellent outcomes, average outcomes and less than outcomes.
I recommend reading all reviews on various physician review websites and avoiding providers with a significant number of justified negative reviews. The absence of justified negative reviews is more important and more telling than having an abundance of positive reviews.
To get a reference in regards to quality of work and background experience continue having consultations until you feel quite confident you found the right provider for your needs.
Simply being bored certified in plastic surgery with an overall good reputation is insufficient to confirm someone has mastered each procedure.
Likewise you may want to leave a review regarding your experience writing it honestly in a professional manner.
I’m sure you would’ve appreciated hearing other patients experiences if they were similar to your own before choosing this provider.
It is important to follow up and let your provider know you’re not happy with the outcome. It’s reasonable to discuss financial compensation in regards to a refund or partial refund for the procedure. Surgeons are not obligated and legally with cosmetic surgery this is probably not considered malpractice.
Still, getting some of your money back is better than having paid for the entire procedure with the outcome you ended up with.
Address the situation calmly in a professional manner and see what you can get.
For a proper second opinion consultation scheduled them as in person consultations avoiding virtual consultations. stay local whenever possible. for a second opinion consultation come prepared with all copies of before and after pictures for both procedures and copies of the operative reports. These are all part of your medical records and available to you from your provider at your request.
Trying to restore tissue with procedures like fat grafting will be difficult and will most likely require multiple procedures.
It’s going to be challenging to make a significant improvement on your current outcome.
If you move forward with seeking a revision for this then consider the above suggestions in regards to finding the most experienced and talented provider.

it appears that you were slightly over-resected and that is why you have some indentation in the lower half of your breasts. you may benefit from having surgery which would entail opening the incision, freeing up the scar tissue in the lower half of the breast area and fat grafting. you don't look like you have much fat to take for fat grafting so that may make it tough. also when you fat graft to a scarred area, probably only 40-60% of the fat is likely to survive. you should know that you may want a second fat grafting procedure several months after the first operation
