Is the gyno surgery worth it at 19 years old?

It is worthwhile to go in for a consult and see if you are a candidate for surgery. If it has not changed since age 14 it is unlikely to now.

The earlier the better. I do not think that gynecomastia goes away after it has been there for two years. There is no reason to have the false hope that it will somehow resolve on its own with diet, exercise, medicine or prayer. If it does not bother you then there is no reason to get it fixed. On the other hand if it is always a source of concern than the earlier you get it corrected the sooner you can move on.
