I have had 180cc saline implants since 1992 and my PS assured me he could remove them via the same scar (under the areolas vs under the breast itself) bc they are so small as well as also perform a full capsulectomy that way. On my BII support group website I have had women saying it was not possible yet I read a popular female racecar driver had hers successfully removed this way in 2018 to not have to encur excessive scarring. She did not need a lift and my PS says I should not either.

In many of our patients, en bloc removal of breast implants with capsulectomy is important and beneficial. Further clinical evidence about breast implant illness is evolving and will continue to guide treatment. I would suggest removal if there is a possibility of foreign body reaction. Whether implants and capsules can be removed through the areola depends on the specialist's surgical technique. Personally for me the best esthetic result of a breast explant is when there is a breast reconstruction immediately after the breast implant removal and capsulostomy. With an L-shaped incision, which for me is the smallest and most discreet incision possible in a breast explant with breast reconstruction. Is important to take into account that after the capsule has been taken out is best to send that tissue to be analyze.
