I am a 37 year old female. Otherwise in great health. I had saline breast implants put in under the muscle almost 15 years ago. About 400-425 cc each implant. I need help deciding: 1. Should I have my prior surgeon do the explant/capsulectomy or another doctor? 2. Do I have explant/capsulectomy now, before moving forward with fertility treatment or after I have baby #2? 3. At what point would be ideal to do a fat transfer?

I am sorry to hear about your symptoms. Implant removal and capsulectomy surgery is not typically covered by most health insurance plans. Check with your health insurance to confirm.
Presently, you still have some breast ptosis or droopiness. If you remove your implants, you may have more loose skin requiring a breast lift. If you're OK with this sagginess, you don't have to do lift. Most surgeons would probably recommend redoing your lift.
