Removing the drain usually doesn't hurt at all (1-2 on a 10 point scale). It's completely worth it, if they are indicated for your surgery. They are annoying and most people are so happy to have them removed that they don't even care about the very momentary mild discomfort.
Drain removal should be relatively painless. One trick is to place some antibiotic ointment at the drain skin interface to keep the drain from pulling on the skin when it is removed.
This is a very common concern! Patients commonly describe the sensation like a sting but only for a few seconds! Good luck!

In my experience of placing thousands of JP drains (usually in the abdomen, I rarely need to use them in breast surgery unless it's for a cancer reconstruction), I'd say that my patients put it at a 0-2; really no big deal. Generally speaking there is minimal pain with drain removal. However, pain is subjective and although most patients tell me that it felt "weird", but not painful, there are some who says it hurts. In any case, it takes seconds to remove the drain and it does not hurt at all after.
