With nonspecific signs like this I would always recommend seeing a plastic surgeon for evaluation. They are likely to recommend you have an ultrasound of your breasts to rule out any more concerning Diagnoses.

You need an examination, so I would suggest meeting with your primary care physician and be checked. You may even need a mammogram. You might also be developing a capsule.

Unfortunately, one cannot determine the nature of your problem without a thorough history and physical examination. A common cause of chronic irritation is inflammation of the permanent "purse string" suture often utilized to maintain areolar diameter during periareolar mastopexy.
Visit a board certified plastic surgeon for evaluation, discussion, and possible treatment.
