Unfortunately, we do not have your preoperative photos to evaluate condition and size of your breast before lift and augmentation. this is one of the most frequent confusions after breast lift (with or without implants) in patient who had large breasts before surgery and did not want reduction. We could only guess that your early postoperative result was satisfactory but, in time, your breast tissue migrated to the lower pole of the breast due to gravity. While implant stays pretty much in the same position where it is placed or, may move up, breast tissue always goes down. The more breast tissue is left after lift or reduction, the sooner will this happen. Some surgeons which favor the use of resorbable mash to support lower pole, claim that result is longer lasting; it is still hard to believe with large breasts? As long as we live in planet Earth and have gravity, this will happen. However, significant reduction of breast tissue performed with lift, and placement of appropriate size implant, may reduce or delay effect of gravity and bottoming out. For above reasons, many plastic surgery would consider correction of bottoming out as a separate procedure and charge surgical fee, possibly slightly reduced.
