I have always been busty but after breastfeeding two children and losing 50 lbs, I’m feeling very deflated and saggy. I’m currently a 36-38 DD. I suspect that I will need a lift and implants to get the upper pole roundness and fullness that I want but I don’t really want to go up a bra size. Is this even possible? Would a breast lift be enough to give the me breasts I had 10 years ago?
Answer: Breast lift Dear Polite641556, my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. In your comments, you mentioned not wanting to increase your breast size. If you prefer not to increase in size, I would advise against using an implant or a larger implant. Consequently, I would recommend only a breast lift. However, considering your age and history, it's unlikely you will achieve significant upper pole fullness with a lift alone. To attain upper fullness, I suggest inserting an implant, coupled with the resection of a significant amount of breast tissue to maintain your current size. This approach, combining breast augmentation with tissue reduction, should also include a full anchor or inverted T breast lift. From the pictures you have provided, you have some striae (stretchmarks) present throughout both breasts, grade 1 ptosis (mild drooping) on both breasts, and slight asymmetry with the left nipple areolar complex (NAC) appearing slightly smaller and lower compared to the right. You are satisfied with your volume. You would definitely benefit from a mastopexy (breast lift) to help improve the perkiness and symmetry of your breasts. Given the significant amount of excess skin present in your case, you would need an inverted-T incision pattern ("anchor" scar)- this would allow for resection of the most skin and the best control over reshaping the breasts to an aesthetically appealing result. Since lack of volume is not the issue, you would not benefit from the implants. The breasts and skin can change a lot over 10 years, especially with all the physiological changes of pregnancy, childbirth, breastfeeding, and weight fluctuations. As a result, your breasts can definitely be made more perky and symmetric, even if they won't be all the way back to the same breasts you had 10 years ago. With all that said, it is always difficult to provide tentative surgical recommendations, as there is only so much information to be gained from the photos and descriptions provided. I recommend for you to come see me or another plastic surgeon for a formal consultation to thoroughly discuss your surgical goals, undergo a formal examination, evaluate all your options (surgical and nonsurgical), and decide on the best procedure(s) for your specific case. You have sufficient volume and don't appear to be a candidate for implants ( which will increase your size).
