It appears that liposuction would be your best option. It will remove some of the fat which will improve the neck contours. Some people are born with wider/fuller necks, and this may be part of the problem here as well.

Hello. Based on your photo, you have moderate to significant amount of fat. Options include liposuction, liposuction with neck lift, and Kybella injections. All these have their advantages and disadvantages. It's best you see a board certified surgeon for in office exam in order to learn what will work best for you. There are factors that can't be obtained from a photograph that only an office exam will obtain (i.e. skin elasticity, your neck anatomy and hyoid position, etc). With a good exam, your surgeon will be able to provide options and set the right expectations for results. Take care.
If your skin elasticity and quality are good, you might benefit from liposuction, which most surgeon are able to do under local anest without need for a hospital visit. There's also a medication called Kybella coming to market for injecting the area of fullness under the chin, it might be a good option for patients.

It would be helpful to see full face and neck pictures from front and sides with the head and neck in a neutral position. The safest and best way to lose body fat is through proper diet and exercise. However, sometimes it is very difficult, even with these measures, to eliminate fat from some areas of the body. Everyone's body stores fat in different areas. Liposuction of the neck with, or without, a neck lift would likely provide the best result, depending on your age and skin elasticity. I would suggest a consultation with a board certified surgeon experience in face and neck rejuvenation to determine the best treatment plan for your individual case. By using your height and weight you can calculate your BMI. This can help you know how close to your ideal body weight you are. If you are above you IBW, then sensible diet and exercise can help you move toward it and the neck roll may decrease. If the neck roll remains even with a normal body weight, some focal liposuction may help with the contour. I would recommend SLIM lipo which is a form of laser liposuction. This is performed under local anesthesia. In addition to melting the fat, the laser also tightens the skin to some degree. The melted fat is then suctioned and removed. Use of a tight compression band for 2 weeks will help the skin re-tighten. good luck.
