Hi, I have had my nose broken twice. Also, have had a septoplasty, turbinoplasty before I was a teen. I'm having difficulty panning down someone who can maintain functionality as well as improve the aesthetic. I'm very much concerned about the tip or the group that my nose has, and the crookedness. Any suggestions? I feel like it weighs down my whole face.

Your previous septoplasty is an issue since we tend to utilize cartilage grafts to support a nasal tip, and this material may not be available from your nasal septum. In that regard, you may require ear cartilage or cartilage from a tissue bank to reconstruct your nose. Answer: You do seem to be a good candidate for a rhinoplasty... What I see is the wide tip, and the wide bridge, and that, as you mentioned, the tip droops and could be elevated. At the same time, attention would be paid to maximizing the airway. Would need to see inside the nose to know what's going on in there. How *much* to elevate the tip, or narrow the tip or bridge, and what to do with the profile, or make other changes? We figure that out with morphs, well before surgery. I'd love for you to see some excellent professionally-designed morphs of what could possibly be done with your nose. Morphs could also help you identify better just what's bothering you, and help you set a goal for the rhinoplasty that's accurate for your tastes. Profile and three-quarter views would be particularly important in morphing your nose. (Side note: in my opinion, morphs should really be done by the surgeon, or he should direct an assistant as she makes the morphs. Morphs should be made with a constant eye to what actually *can* be done in surgery, for that particular nose, and the surgeon has that information and judgment best.) Finally, remember that rhinoplasty is an exquisitely difficult operation to get right, and you should only have surgery if you are able to make yourself very confident in your surgeon's skills. The changes that your nose needs require advanced techniques, and skill that most plastic surgeons don't possess with expertise.
