Swelling and bruising can last 1 to 2 weeks after lip filler injections. To diminish the bruising, I recommend using LightStim LED or, in more significant cases, a vascular laser or IPL treatment. Arnica with Bromelain (""Arnika Forte"") can also be taken to help speed up the healing. If you're unhappy with your results, you can also have the filler dissolved with hyaluronidase and try a different filler and/or technique next time. You can use a cool compress or Arnica for relief. If the area does not improve, gets worse, or becomes incredibly painful I would contact your injector!

It appears from your photo that Juvederm created a bruise to the area and it should resolve in about one week. Any injection of hyaluronic acid can potentially bruise and it is always more of a chance if you are on blood thinners. Swelling and bruising are common after lip injections and can take up to two weeks to resolve. You can use intermittent cold compresses to the area. Some patients also find the use of Arnica to be helpful. It appears you have a bruise which should resolve with time.
From the submitted photo, it appears more as a bruise. Lips are very vascular, so they bruise easily and can repair quickly. Nonetheless, it will likely take 1-2 weeks for that to disappear. Avoid all blood thinners (like non-tylenol pain meds, red wine, garlic, ginger, fish oils, etc.) and take arnica montana to help it to go away faster and not re-bleed.
Lumps are normal after filler. As your RN stated, massaging is the best way to get rid of lumps and they will subside. The lumps may have moved therefore appearing in other areas. Continue to massage to break down the filler as much as possible.
