Although you only provided one photo while smiling, there clearly seem to be two components to the wrinkling you have: the dynamic crow's feet emphasized by smiling, and a more generalized crepey appearance overall within the same aesthetic unit and extending down the cheek somewhat, which I suspect is quite evident at rest (static crinkling), as well. The latter, the static component, can be addressed with the appropriate use of a filler designed for stimulating bio-remodeling, such as Skin Boosters, which I would use in my Israel satellite facility, where it is regulatory agency approved and Belotero Balance, which I would choose in my Upper East Side Manhattan practice. Fanned through crow's feet and upper outer cheek region, or injected in microdroplets in a grid pattern, this kind of treatment typically results in immedate, improved skin hydration and new, native collagen, both of which lead to skin tightening. Following this, Botox injected into the crow's feet regions themselves will handle the motion-related wrinkling you dislike. Finally, an at-home regimen of sunscreen and prescription-strength topical retinoids and alpha hydroxy acids should help to maintain and prolong the improvements achieved with the above and with time improve the overall clarity, tone and superficial texture of your skin. Make sure you consult with a board certified aesthetic dermatologist who can help you with each of these issues. Best of luck to you.
