I am a healthy weight (BMI 21) but my jawline isn't as sculpted as I would like it to be. I know a lot of chin/jawline procedures target, fat, but one thing I noticed is when I put my tongue on the roof of my mouth, my double chin disappears. Could it be that I have a different anatomical issue (like muscular) or can it still be fat? I am asking because I don't want to pursue a treatment that targets fat if this is not the primary issue and won't give me the results that I am looking for.

Yes, putting your tongue on the roof of your mouth can pull on the tissues in your neck and tighten the area.  This doesn't necessarily mean that your double chin wouldn't be treated well with liposuction, but you should be seen in person to evaluate what is causing your double chin and whether liposuction, jawline augmentation, or a procedure that targets deeper structures like a short scar neck lift (or a combination of any of these) would best address your concerns.

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). A full set of pictures From all angles are required to make a determination about how best to proceed. It is important to understand there are two compartments a fat in the neck, and they're located both above and below the platysma muscle itself. Liposuction can accomplish only removal of the fat deposits above the platysma muscle, while a surgical neck lift is required to remove the fat deposits below the platysma muscle which also includes a platysma plasty to significantly improve the jawline

It is very rare for those who seek set removal for a double chin to actually have a problem with fat distribution. What they tend to have in common is a Mandible that is small, or lacking in the projection in comparison to the Maxilla.
Fat removal can give impressive results and certain visuals done by a sufficiently experienced provider using Liposuction. Liposuction results very dramatically among different providers are the number of providers really mastered the procedure and can make accurate predictable assessments and deliver consistent quality outcomes time and time again are in a minority.
