69F, 5'5", 120lbs, what could be causing swollen face 16 months after facelift? (Photos)

I am a 69yr old woman, 5'5", 120lbs. I had a deep plane facelift and erbium laser 16 months ago (3/2024). My lower face (below cheeks) persists in looking big. It is not swollen glands, as a scan ruled that out. I have lost 2 lbs since the surgery, so it's not that either. What could be causing this?

It appears from the photographs that you share that there may be some protuberant buccal fat that could be removed. Check with your surgeon and see if buccal fat was removed and if not, if it could be.

Based on your post of photos, the swelling seems to be out of the normal. I would recommend a full medical workout with additional blood test to rule out a systemic disease. Seek good primary care or internal medicine doctor to rule out unknown issues occurring in your whole body. Good luck.
