Do I need a clitoral hood reduction with my labiaplasty? (Photo)

Hi and thank you for sharing photos. All depends on how much labial tissues is going to be taken. Nevertheless Clitoral hood will show more prominent to an extend unless a clitoral Hoodectomy is done. If was your surgeon, I wouldn’t go with the Labiaplasty without a clitoral Hoodectomy done on the same time.

I recommend a small clitoral hood reduction if you decide to do a labiaplasty. Based on your photo, I think your clitoral hood will look a little large if you do only a labiaplasty. Right now, both the clitoral hood and the labia are balanced, meaning they match each other in size. If you reduce one area, it will look better if you reduce the other area as well.

Thank you for your questionClitoral hood reduction is an inaccurate term which in most cases would more accurately be termed reduction of prepuce. There are actually two pair of inner labia: a lower pair which inserts into the clitoris and an upper pair called prepuce which can insert into and distort the lower pair. The prepuce should be first reduced reading a "bridge of the nose" appearance so as to free up the lower pair and reduce the bulk effect. The hood is just the tissue immediate covering the clitoral glans (often referred to as the button). In your case there is excessive prepuce and most likely your surgeon was not well trained in handling the prepuce. I don't believe you will be happy if it is left there. Definitely keep looking until you find a surgeon who knows how to handle both the labia and prepuce. DO YOUR HOMEWORK. Get educated and check out the video below to learn more.

Your clitoral hood looks redundant. I think you will benefit from a clitoral hood reduction with labiaplasty. If you do not do it, the clitoral hood will be protruding like a small penis. You will probably need a V-Y wedge labiaplasty with clitoral hood reduction.

Nearly every labiaplasty patient benefits from a clitoral hood reduction. This brings balance to the top in the middle portions of the vulva so that the shape is uniform throughout. It’s likely that some inexperienced surgeons have fear about the innervation and sensation in the area but for a labiaplasty surgeon with good experience this is a non-issue And a hood reduction can be very safely performed with zero chance of nerve injury. In my practice, nearly every labiaplasty patient has a hood reduction.

You have a large, wide protuberant clitoral hood, which should be reduced at the same time as your labiaplasty. Your hood would look much larger if it is not done. A labiaplasty with a clitoral hood reduction can reduce your hood size. However, it must be performed by a surgeon with experience. One of the two most common labiaplasty techniques is the central wedge technique, which I invented in 1995 and published in the plastic surgery textbooks. It is also known as the "V" or wedge technique. Gynecologists and most plastic surgeons perform a labiaplasty very differently. They essentially trim the labia minora (inner vaginal lips) and leave a long suture line instead of the normal labial edge. Their technique is the same whether a scalpel or a laser is used and can lead to scallops, asymmetry, and over-removal. In contrast, the central wedge removes triangles of tissue and bring the normal edges together. Thus, the normal labial edges, normal color, and normal anatomy are preserved, but the darkest labial tissue is usually removed. There is rarely a sign that the woman has had surgery.

As I stated, your clitoral hood is wide, thick, and protuberant. It can be decreased somewhat by reducing the sides. I pioneered a clitoropexy with clitoral hood reduction in which the clitoris is pushed closer to the pubic bone and the width, thickness, length, and protuberance of the clitoral hood can be decreased. With this procedure, I lift up the clitoral hood skin, move the clitoris closer to the pubic bone, thin the tissues under the clitoral hood skin, and then remove a large amount of excess skin. This results in a dramatic decrease in the length, thickness, and protuberance of the clitoral hood, so that the hood is now within the labia majora when the woman stands. I have performed this procedure over 350 times with outstanding results. I have not published this technique, so no one does it like me.​

No matter the technique, an inexperienced or unskilled surgeon can lead to a high rate of complications, chronic scar discomfort, labial deformities, and further surgery.​​ Approximately 40% of the female genital surgeries that I perform are reconstructions of other women's botched labiaplasties and hood reductions.
