Do I need a clitoral hood reduction with my labiaplasty? (Photo)

Hello, I had a consult for a labiaplasty. I liked the doctor but she recommend not to get a clit. Hood reduction just a labiaplasty but Im reading that not getting it done at the end would make the clit hood look bigger. Should I just keep looking? Or would it look fine without one?

Hi and thank you for sharing photos. All depends on how much labial tissues is going to be taken. Nevertheless Clitoral hood will show more prominent to an extend unless a clitoral Hoodectomy is done. If was your surgeon, I wouldn’t go with the Labiaplasty without a clitoral Hoodectomy done on the same time.

You have a large, wide protuberant clitoral hood, which should be reduced at the same time as your labiaplasty.  Your hood would look much larger if it is not done.  A labiaplasty with a clitoral hood reduction can reduce your hood size. However, it must be performed by a surgeon with experience. One of the two most common labiaplasty techniques is the central wedge technique, which I invented in 1995 and published in the plastic surgery textbooks. It is also known as the "V" or wedge technique. Gynecologists and most plastic surgeons perform a labiaplasty very differently. They essentially trim the labia minora (inner vaginal lips) and leave a long suture line instead of the normal labial edge. Their technique is the same whether a scalpel or a laser is used and can lead to scallops, asymmetry, and over-removal. In contrast, the central wedge removes triangles of tissue and bring the normal edges together. Thus, the normal labial edges, normal color, and normal anatomy are preserved, but the darkest labial tissue is usually removed. There is rarely a sign that the woman has had surgery.

As I stated, your clitoral hood is wide, thick, and protuberant. It can be decreased somewhat by reducing the sides. I pioneered a clitoropexy with clitoral hood reduction in which the clitoris is pushed closer to the pubic bone and the width, thickness, length, and protuberance of the clitoral hood can be decreased. With this procedure, I lift up the clitoral hood skin, move the clitoris closer to the pubic bone, thin the tissues under the clitoral hood skin, and then remove a large amount of excess skin. This results in a dramatic decrease in the length, thickness, and protuberance of the clitoral hood, so that the hood is now within the labia majora when the woman stands. I have performed this procedure over 350 times with outstanding results.   I have not published this technique, so no one does it like me.​

No matter the technique, an inexperienced or unskilled surgeon can lead to a high rate of complications, chronic scar discomfort, labial deformities, and further surgery.​​ Approximately 40% of the female genital surgeries that I perform are reconstructions of other women's botched labiaplasties and hood reductions.

I would suggest that you visit with a plastic surgeon or urogynecologist in your area who does a lot of these procedures for an in person exam and a frank discussion of the risks and potential complications. Thank you for your questionClitoral hood reduction is an inaccurate term which in most cases would more accurately be termed reduction of prepuce. There are actually two pair of inner labia: a lower pair which inserts into the clitoris and an upper pair called prepuce which can insert into and distort the lower pair. The prepuce should be first reduced reading a "bridge of the nose" appearance so as to free up the lower pair and reduce the bulk effect. The hood is just the tissue immediate covering the clitoral glans (often referred to as the button). In your case there is excessive prepuce and most likely your surgeon was not well trained in handling the prepuce. I don't believe you will be happy if it is left there. Definitely keep looking until you find a surgeon who knows how to handle both the labia and prepuce. DO YOUR HOMEWORK. Get educated and check out the video below to learn more.

Hello, I definitely recommend clitoris aesthetics. 70% of patients require clitoris aesthetics. I recommend having it done by an experienced surgeon with the right technique. Nearly every labiaplasty patient benefits from a clitoral hood reduction. This brings balance to the top in the middle portions of the vulva so that the shape is uniform throughout. It’s likely that some inexperienced surgeons have fear about the innervation and sensation in the area but for a labiaplasty surgeon with good experience this is a non-issue And a hood reduction can be very safely performed with zero chance of nerve injury. In my practice,  nearly every labiaplasty patient has a hood reduction.

Hello , looking at your picture, I can say that adding clitoral hoodoplasty to your labioplasty operation would be much better. If labioplasty is performed alone, the clitoris will appear very large and not suitable

Thank you for your question and photos. Based on these photos, you definitely will need a clitoral hood reduction as well. The surgeon you consulted for this procedure is either a novice or not experienced in female genital cosmetic rejuvenation. See a board certified plastic surgeon or gynecologist who have extensive experience with these types of surgeries. These surgeries are done as outpatient and with local anesthesia with minimal oral sedation. General anesthesia and surgery done in the hospital are done by surgeons who are learning this procedure. Remember, not every plastic surgeon or gynecologist is an expert in this procedure. Your best chance for a good result and in the end the, less costly, is to have this procedure done correctly the first time. Good luck.
