I had labiaplasty at age 18. It changed my life and I was pleased with my symmetric results for 8 years until I recently stopped birth control temporarily. My labia grew back and became dried out. back on birth control for good. The surgeon I went to in 2013 is now out of state and no longer does labiaplasty. What should I seek in revision? I was able to obtain my pre surgery photos (included) and notes. photos now at 28 after the hormonal shift showing the regrowth plus a little flap/hole.

Answer: Revision of previous labiaplast Thank you for your questionThere are two pair of inner labia: A lower pair that inserts into the clitoris and an upper pair called prepuce, that forms the hood (the skin directly above the clitoris) that can insert into and even overwhelm the lower pair. In your photo there is asymmetric excessive prepuce on both sides especially on the right side. For optimum aesthetic results, this excess tissue needs to be removed so as to free up the lower pair for reduction. We look to get a "bridge of the nose " appearance on this upper section. There are several ways published for reduction of the lower pair which give different appearances and each surgeon has their own preferences. What is important is what you prefer. Most of my patients request the natural edge technique. You need to do your homework and make sure your surgeon is well accomplished first in how to reduce the prepuce and second on how to reduce the lower pair. Answer: Re-do previous labiaplasty As the inventor and innovator of the wedge technique in 1994, I reconstruct many women who have had labia reductions by other surgeons, both wedge and trimming techniques. You had a trimming labiaplasty and have had some re-growth, which resulted in asymmetry, excess clitoral hood, and scalloping. If you wish symmetry with elimination of the scallops and a natural appearance, I would do a wedge reduction on the right and probably a clitoral hood flap to build up the left and remove most of the scallops. I would reduce your hood also. The reconstruction is much more difficult than a primary labiaplasty and should be done by a plastic surgeon with extensive experience in labia reconstruction. I published the first and most extensive paper on labia reconstruction in the prestigious journal "Plastic and Reconstructive Surgery" about ten years ago. In that paper, I presented the labia reconstruction with the clitoral hood flap in which I take the skin form the sides of the clitoral hood to make a normal labium. You can have a much better appearance by a very skilled reconstructive labiaplasty surgery. Approximately 40% of the surgeries I perform are revisions of other surgeons' labiaplasties.
