So I am just over 6 months BA. They are softening and slightly dropping, but my main concern is where my nipples still are. My surgeon didn’t touch my nipples and they are still in a downward position. Is this going to change or do I need another surgery? I feel they should be more centered by now. Thank you in advance.

Don't go bigger. Your tissue is not going to support the additional weight over time and you will bottom out, even if mesh is used. Ideally, you should go smaller.

You failed to mention that you had a lift done as well. some of the differences are due to the lift. I would suggest going back to your surgeon and discussing your concerns and options.
