If your nipples are still pointing downward after six weeks or so, you should definitely consult with your original surgeon. A revision may be needed in the future.

You chose large implants and they may have settled as much as they can. Smaller implants would have dropped and settled better. Your nipples can now be raised with a lift if you want, or you can go with smaller implants that will fit your frame and tissue better.

Appears you requested oversized implants thus causing upper pole of the breasts over inflation appearance with N/A downward positioning. Only full revision with lift can effectively improve.
