Hi! I had a breast lift with implants and strattice (built in bra) in 2015. Now almost 7 years later my breast are once again really low and sagging, yet my nipples are fine. I lost all upper pole. Considering removing implants and getting lift again but looking at before and after pics, this won’t help. What other options could help this problem?
Answer: Revision Hi, thank you for sharing your question. You are right in saying that removing the implants and doing a lift may not give you what you are looking for with regard to upper pole fullness. It seems that you may have some glandular ptosis which will require a revision lift without moving the nipple position. If you don’t have a capsular contracture, the implants can stay where they are. An in person evaluation with a board certified plastic surgeon will help give you the best answers and treatment plan.

Answer: Upper pole of breasts Thank you for your inquiry! To create upper pole breast volume, you need to put something in there: an implant or fat. We would love to sit down with you and review your options. Schedule a consultation with a Board-Certified Plastic Surgeon to discuss your goals.

Answer: Re-Lift You may need another mastopexy procedure to lift again. With large implants, recurrent ptosis is not uncommon, even with Strattice support, due to the weight of these implants.

Answer: Upper pole From your photos it appears a revision is indicated. You could consider a lift with smaller implants to give a more natural appearance and lifted look with the upper pole fullness you are wanting. Begin by visiting with a board certified plastic surgeon near you for a consultation.

Answer: Explant with Bellesoma Method The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
