Will a facelift help my nasolabial folds and can my nose be fixed? (Photos)

I am interested in getting a facelift, unsure about the best type/ option. I am very unhappy with my nasolabial folds and jowls. I would also like my nose repaired. I had surgery done in 2006 for a deviated septum, but was injured after the surgery, which￼ caused some curving of my septum tissue (see picture). I’m concerned about using only filler for the folds. I want something that looks a little bit more natural. Perhaps that transfer is a better alternative?

Thank you for your question.  I have been in practice for over 30 years.  You are in excellent candidate for a facelift as well as a revisionary rhinoplasty and fat transplantation to the nasolabial folds and marionette lines.  I often combined facelifting and rhinoplasty. You may benefit from a rhinoplasty to refine the tip of the nose and correct the dorsal irregularities. You would also benefit from a facelift and browlift. You may also benefit from an eccentric lip lift, not a bullhorn lip lift. This does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.

Based on your concerns, you would likely be a suitable candidate for a facelift with neck lift combined with fat grafting, which can improve jowls, soften nasolabial folds, and enhance the neck for a natural, long-lasting result. For your nose, a revision rhinoplasty may be considered to address the curvature.

For best overall result and longevity of results, some treatment of the SMAS layer of your face needs to be done. There has never been any scientific evidence that has proven that among the different options for treating the SMAS:  high SMAS, extended SMAS, SMAS plication, SMAS Delta lift, Deep Plane, etc, gives patients a better result, or a longer lasting result.  Moving the SMAS vertically/posteriorly redistributes tissues to their correct, youthful position and improves facial shape and contour.  The skin can then be re-draped over this improved facial shape without much tension on the skin. If you wish to avoid facial fillers, some micro grafting with fat may offer a more permanent solution for the folds beside your nose. It has the benefit of being a permanent solution if the fat is retained and has potential downsides of contour irregularities due to loss of some of the fat. Correction of your nasal septum could also be achieved at the same time.
