Thank you for your questionIts a difficult yet common problem that you have.I typically stage the procedures as you don't want perform a full thigh lift with the possibility of a local infection. First thing to do is full control of the hydradenitis. Once controlled surgical excision of the hydradenitis regardless of the cosmetic appearance. If there is no recurrence of the hydradenitis after one year then may consider cosmetic surgery. You can not do cosmetic surgery with the infected hydradenitis
