Dr. Bruneteau is from Omaha and he attended medical school at the University of Nebraska Medical Center. In addition to his general surgery residency at the University of Iowa he also completed a residency in plastic surgery at the University of California. Dr. Bruneteau finished a fellowship in micro vascular surgery at the University of California and a fellowship in craniofacial surgery at Harvard Medical School. Dr. Bruneteau returned to Omaha in 1990 and has been in practice at ASI since that time. His decision to become a plastic and reconstructive surgeon was influenced by his desire to help people. Plastic surgery allowed him to do this in a way that used artistry and creativity.

My experience with Dr. Bruneteau (and his staff) so far has been amazing! I lost about 275 lbs over a three year period. Obviously, I have a lot that could be done to help change my appearance when it comes to removal of loose skin. I had my initial consultation with Dr. Bruneteau before I had met my goal weight just because I wanted somebody to educate me on the best approach for someone in my category. And I knew that Dr. Bruneteau had worked with patients who had lost the kind of weight I had and more. I have to share that my first in-person impression of Dr. Bruneteau is something I will always remember, and for a pretty simple gesture. I was sitting in the waiting room about 10 minutes before my appointment. He was passing through the walkway and he looked over and said hello. Then he said "we're meeting in a few minutes, right?" I said "yes, sir!". He then walked over, shook my hand, and introduced himself and said "see you shortly". This may not sound like that big of a deal. But he didn't know me, had never met me, didn't know anything about me at this point. And I'm sure he was busy. But he made the time to come over and introduce himself when he could have just waited to do that once my appointment started. So, that was my first impression of Dr. Bruneteau. And that is a true representation of who he is as a person and a doctor. During my consultation, he answered every one of my questions, asked me a few questions, and gave me detailed information to educate me on what he felt would be the best strategy for me. He took all of the time that I needed and I never felt rushed. He encouraged me to keep losing as much weight as I can and come back to see him in 6 months. So, I went back 6 months later (in June of 2024), and he approved me for a 360-lower-body lift that day. On the day of the surgery, I was very well taken care of by the nurse assigned to me. Everyone involved was wonderful. The surgery itself went well. I believe it took just over 3 hours. And Dr. Bruneteau removed 31.5 lbs of loose skin from my mid-section (front and back). I came back the next morning for my first follow-up, and that was when he took my binder off for the first time so that I could see the initial result. And I cried because I was so happy. He did an amazing job! I could not be happier! I'm now 4-weeks post-op and it continues to look better and better as the swelling continues to go down. I've seen Dr. Bruneteau every week since the surgery for follow-up appointments to make sure healing is going well and that any other hiccups are addressed. The aftercare by Dr. Bruneteau and his team has been outstanding. And when I've called the office a few times with questions, they are always courteous and are able to help me in a timely manner. I am so impressed with this place. I plan to have more work done as soon as Dr. Bruneteau clears me to do so, hopefully in the early part of 2025. I want to do my upper-body and thighs next. The last thing I'll say for now is this... I have spent most of my life being ashamed and embarrassed about how I look. And any time I've had to go to see a doctor and have to remove my clothes, I feel extremely uncomfortable and worried about judgment. I know they've seen it all. But that's how my brain works... Anyway, I want to say that, even in the most vulnerable moments during this process, I have always felt comfortable with whoever I was working with that day. You can tell that, as a business, the entire staff goes the extra mile to make sure their patients feel comfortable. And that is not an easy accomplishment. Thank you, Dr. Bruneteau! Dr Bruneteau does great work, to the point. He is confident in what he can do. All my questions were answered. Pre op exam I felt was rushed with Dr and there was a long wait. There are a lot of appointments which is good to make sure everything is healing. You will need to adjust your work and kids schedule. Post op visits were quick and Dr was really happy how my procedure went and confident that I'll love the outcome. Updated on 14 Jun 2023: Surgery was the only option for me. I had a 3 inch long by 1 inch wide diastasis recti, small hernia, and 3 to 4 inches of extra skin from having my three kids. Certain exercises were tried and over the years my hernia was getting bigger.

It’s not unusual to see changes in the nose in the immediate post-operative period. This occurs for a variety of reasons including the healing process and the resolution of swelling. When surgeons perform rhinoplasty they utilize a series of detailed maneuvers on the underlying bone and cartilage. In the initial post-operative period, these changes are often obscured by inflammation and swelling. In some cases, as swelling resolves and nasal contour improves, the nasal tip may actually appear to drop. The nasal tip should be more or less stable after the resolution
Optimal results for rhinoplasty surgery are dependent upon an accurate assessment of the patients anatomic deformity. This requires a physical examination by a surgeon with experience in this area. In most cases a relatively accurate assessment of the nasal anatomy can be made prior to surgery when patients have prominent nasal structures. Unfortunately, even when an experienced surgeon examines a patient, it can be difficult to distinguish thickened nasal skin from excess cartilage. Under these circumstances, appropriate adjustments can be made in the operating room to optimize the surgical result. When these steps are taken clinical results are excellent and levels of patient satisfaction are high.
Stiffness of the nose and tip can occur following rhinoplasty surgery. This phenomenon is more likely when extensive dissection is performed at the junction between the nasal cartilage and lip soft tissue. Although it’s impossible to know exactly what was done in your case without an operative report, it appears that both bone and cartilage were removed from the distal nose. In addition, it appears that the depresser nasi muscles were detached. These maneuvers involve extensive soft tissue dissection which may be the source of your symptoms. Under these circumstances, prolonged swelling, scarring and structural changes can result in the symptoms that you describe. Structural changes from the use of cartilage grafts and non-absorbable sutures can cause temporary immobility of the soft tissue. A more likely cause is scarring and prolonged swelling. All of these problems should resolve with the passage of time. In some cases, resolution of lip tightness can be facilitated with gentle massage. It’s important to realize that resolution of these problems may take up to a year following surgery. In the interim it’s important to be patient and maintain good communication with your plastic surgeon. Your surgeon should be able to reassure you and alleviate your anxiety.
The suture remnant that you describe is most likely a non-absorbable suture. If left alone this suture will probably work its way to the surface and can be easily removed. Unfortunately foreign material does have the potential to become infected. For these reasons, we generally recommend that these suture remnants be removed as soon as possible following surgery.
Rhinoplasty surgery can be performed through either an open or closed approach. With both of these techniques incisions are made on the inside of the nose along the nostril rim. When a closed approach is utilized no additional incisions are made. The surgeon is able to operate through these nostril incisions but doesn’t have direct visualization of the underlying anatomic structures. In contrast when an open approach is utilized a small incision is made across the columella that connects the two nostril incisions. This small incision tends to heal nicely and is almost imperceptible in most cases. This approach gives the surgeon the ability to lift the skin off the underlying bone and cartilage and directly visualize these anatomic structures. The approach utilized depends upon a variety of factors including the patient’s anatomy, aesthetic goals and the surgeon’s preference. Historically, the vast majority of rhinoplasties were performed with a closed technique. This has gradually changed over the years and now most rhinoplasties are performed with an open approach. This is probably due to the fact that surgeons can more easily visualize the underlying anatomy when this approach is utilized. It’s important to understand that most surgeons utilize both approaches. Most surgeons agree that the surgical approach utilized is less important than the surgeon’s skill. For this reason, it’s important to consult a board certified rhinoplasty surgeon with experience in this area.
