Implants bottomed out twice - why?

I had 334 silicone put in. They bottomed out right away. So we put in 280 and surgical mesh and redid the pockets. Bottomed out again. They feel like they are completely floating free in there. Painful sleeping on side. Bra 24/7. Doc wants to restich the bottom again in a third surgery. Said not much to do for sides. I have a narrow rib cage. 120 pounds and 5'3". Why did this happen?
