A little over over one year, postop implant revision and lift. My results look deformed. Is there a way to fix this? (Photo)

your issues are relatively small and can be fixed. Replacing with smaller implants, securing the pocket, and managing the skin envelope with proper skin excision will give you a smaller, more aesthetically pleasing shape. Find a new surgeon.

My advice would be to downsize your implant and shorten the nipple to breast crease distance. I'd suggest converting from a vertically based mastopexy to a full scar or wise pattern incision to allow removal of skin from the lower pole. I'd reconstruct the inframammary breast crease with sutures and mesh: Galaflex or Durasorb.

You will need smaller implants placed with an inframammary incision to reposition the inframammary fold higher and remove any of the excess skin. The contour deformity you see is the old inframammary fold.

I would be interested I knowing the size implants you agreed to versus what you have. You can always downsize your implants and the skin under the breast tightened. That would involve extra scars however

Thank you for your question. A large portion of my practice is dedicated to revision breast surgery, and I have been in practice for 28 years. I have a little bit of experience to address your concerns. You are a relatively straightforward fix. I would perform an implant exchange to a smaller implant, internal bra-capsulorrhaphy, and revision of the inferior portion of your mastopexy-breast lift. I wrote the very first paper on capsulorrhaphy over 20 years ago. You should be able to have an excellent result with an attractive breast. Best wishes to you.

Your implants are too large for your frame and tissue and are bottoming out from the weight. I would suggest going smaller, at least 100 cc's smaller, and tightening your inferior pocket back to its original location. Mesh might be needed for additional support.

When I see a posting like this it always boggles my mind. I do not understand why a surgeon would choose much larger implants than the patient desires and documents. Occasionally an implant desired will be larger than can be accommodated and this is usually discussed upfront.

Having said this, this is a correctable problem by redoing the breast lift and downsizing your implants combined with capsule correction

I would certainly seek in person consultations for a valid assessment.

From your photos it appears that you have the correct disgnosis. I believe the only way to correct your issues (botteming out) is to redo the surgery with smaller implants. Support of the bottem of the breast may also be necessary.

Unfortunately, it appears that your implants have slightly bottomed out, and you have some contour deformity. In order to orrect this, you would need exchange of implants to a smaller size and a revision lift, which would result in both a vertical and a horizontal scar beneath the breast fold.
